You can submit an application for a CHRS through the Court. The fee is $95 per search.

For the Court to complete a CHRS, you must provide the full name of the individual, including the middle initial, and the full date of birth. Partial names or dates of birth are not accepted.

You can request a CHRS online or by mail.

Mail completed CHRS application to:

Criminal History Record Search Unit

The results of the search can be sent by:

• Email (MUST include an email address on the application form)
• Mail (MUST include a self-addressed stamped envelope.)

Upon receipt these requests are also processed within the next business day.

Fees are payable by check or money order made out to NYS Office of Court Administration.
